I recently received a postcard from your office stating that I failed to appear for Jury Duty on 02-03-2000....

... On October 10, 1997, I was first summoned for jury duty in the county of Riverside and I submitted the completed Juror Affidavit Questionnaire with a statement from my physician requesting a permanent excuse from jury duty. That was for Badge #174780362, Group #430.

On October 21, 1997, I received a postcard from your office stating that "Your request to be EXCUSED from jury duty is granted."

In April of 1999, I was again summoned and again submitted the request for permanent relief (submitting copies of the original affidavit and the physician note, I believe). On April 9, 1999, I again received a postcard from your office stating that "Your request to be EXCUSED from jury duty is granted."

Naturally, I assumed I was not being summoned anymore because of this permanent excuse. So, once again, ... I am requesting to be permanently excused.

Is there anything else I need to do so that I don't keep getting summoned or should I just reply like this each time?
